Root Cause Analysis Versus Shallow Cause Analysis: What's the Difference?

Program Description
This presentation provides an overview of the processes and tools commonly used in the health care marketplace to solve undesirable outcomes. Most refer to themselves as "Root Cause Analysis." Are they? What is the difference between troubleshooting, brainstorming, problem solving and root cause analysis? Are the outcomes different when we use The 5-Whys, The Fishbone Diagram or a Logic Tree? Can deficiencies in our approach to RCA increase the risk of harm to the patient? These questions will be discussed in depth and contrasted using a common example to determine if we are using Root Cause Analysis or Shallow Cause Analysis techniques.

Learning Objectives
At the completion of this program, the participant will be able to:
  1. Identify the pros and cons of troubleshooting, brainstorming, problem solving and RCA
  2. Contrast the technical differences between using the 5-Whys, the Fishbone diagram and a Logic Tree
  3. Describe the physical, human and latent roots of undesirable outcomes
  4. Discuss the human factors contribution to causing undesirable outcomes
  5. Explain when it is appropriate to use which processes and tools for the situations at hand

Robert Latino, EVP, Reliability Center, Inc., Hopewell, VA
Robert J. Latino is executive vice president of Strategic Development for Reliability Center, Inc. (RCI). RCI is a reliability consulting firm specializing in improving equipment, process and human reliability. Latino has been facilitating RCA & FMEA analyses with his clientele around the world for over 20 years and has taught over 10,000 students in the PROACT® Methodology. Latino is co-author of numerous seminars and workshops on FMEA, Opportunity Analysis and RCA as well as co-designer of the national award winning PROACT Suite Software Package. Latino received his Bachelor's degree in Business Administration and Management from Virginia Commonwealth University.

How the Webinar Will Work

Substitution, Transfer and Cancellation Policy
Cancellations received in writing up to five business days prior to an event will be given a full refund less a $50 processing fee. No refunds will be given for cancellations received less than five business days prior and day-of-program no-shows. Substitutions are accepted.
