The Medicare Wage Index has been an important part of the Medicare inpatient prospective payment system for a long time. However, now, the wage index plays an even larger part in the calculation of Medicare payments made to providers. A Critical Access Hospital (CAH) is often located in a rural part of the state, and it provides short-term hospital stays when someone has a severe injury or illness, an urgent medical condition, or is recovering from surgery. A freestanding critical access hospital allows for a measured transition toward an integrated multi-service health care facility. CAHs help preserve local health services and the unique qualities of the small town.

Two out of three patients receiving care at a Wisconsin hospital are covered by either Medicare or Medicaid (BadgerCare). Both the federal Medicare and state-administered Medicaid programs underfund Wisconsin hospitals, causing a cost-shift onto everyone else who receives care at a hospital. This underfunding and resulting cost-shift has been called a “Hidden Health Care Tax” by Wisconsin businesses. Between Medicare and Medicaid, the government programs underfund Wisconsin hospitals by $3 billion a year – for hospital services alone.
