WHA’s Eighth Annual Post-Acute Care Seminar Agenda Announced

Registration now open for the virtual event
WHA’s eighth annual Post-Acute Care Seminar, which will focus on innovative care models, will be held August 6 from 10 a.m. to 2 p.m. Attendees will participate in the new format virtually.
 
Attendees will hear about Froedtert Health’s groundbreaking work with Homecare Hub to build a local network of group homes and create a search engine that helps locate appropriate discharge options for patients needing post-acute care and residential services in southeast Wisconsin. SSM Health will present on a new service being offered to appropriate patients in their Madison market: Recovery Care at Home. This program, launched earlier this year in April, is the first of its kind in Wisconsin and provides patients with advanced, personalized care within the comforting walls of their homes.
 
The virtual event will conclude with a panel of hospital leaders from around the state who are focused on securing appropriate post-acute care services for hospital patients who are ready to be discharged. Panel members will discuss some of their key practices, share their insights and irritations, and describe actions they’re taking to support their patients and organizations.
