Program Description


WHA’s 8th annual Post-Acute Care Seminar, which will focus on innovative care models, will be held August 6 from 10 am to 2 pm. Attendees will participate in the new format virtually.
 
Attendees will hear about Froedtert Health’s groundbreaking work with Homecare Hub to build a local network of group homes and create a search engine that helps locate appropriate discharge options for patients needing post-acute care and residential services in southeast Wisconsin. SSM Health will be presenting on a brand new service being offered to appropriate patients in their Madison market: Recovery Care at Home. This program, launched earlier this year in April, is the first of its kind in Wisconsin and provides patients with advanced, personalized care within the comforting walls of their homes.
 
Finally, a panel of hospital leaders from around the state who are focused on securing appropriate post-acute care services for hospital patients who are ready to be discharged will discuss some of their key practices, share their insights and irritations, and describe actions they’re taking to support their patients and organizations.
