Advocate Health is committed to implementing the 4Ms framework across all inpatient units and achieving Age-Friendly Health System recognition from the Institute for Healthcare Improvement (IHI) at all 25 of our Midwest hospitals by 2025. To date, 24 hospitals have earned this recognition, with the 4Ms successfully implemented on 160 units. They are actively working to implement the framework on the remaining 30 units and aim to secure recognition for our final hospital by October 2025.

Over the past two years, eight of their Wisconsin hospitals have showcased their Age-Friendly efforts during Wisconsin Advocacy Day. Additionally, they submitted applications for the Wisconsin Hospital Association’s Patient Safety Awards in recognition of their efforts. Each year, one of these hospitals has been honored with the award. Our aim is to continue to enhance care and outcomes for older adults.
