WHA is partnering with Rush University's Center for Health and Social Care Integration (CHaSCI) to present a webinar series focusing on reducing 30-day readmissions through the integration of practices, policies and models that expand access to social care. Join us to gain a deeper understanding of health-related social risk factors, learn clinical skills to address social care and tangible steps to integrate social care into transitional care planning. For more details and information, see the program flier.
