Improving Care for Older Adults: Introducing the 4Ms Framework for an Age-Friendly Hospital

WHA, with sponsorship provided by the Wisconsin Office of Rural Health, is offering the following webinars for critical access and small rural hospitals on how to become an age-friendly hospital.
 

  • Introducing the 4Ms Framework for an Age- Friendly Health System
Becoming an age-friendly health system entails reliably providing a set of four evidence-based elements of high-quality care, known as the “4Ms,” to all older adults in your system: What Matters, Medication, Mentation, and Mobility. The Rush Center for Excellence in Aging will facilitate the 4M training and two follow-up technical assistance meetings to assist hospitals in moving forward with implementing the framework. In addition to the expertise offered by The Rush Center for Excellence in Aging, participants will also learn from and with hospitals in Wisconsin who are on the path to becoming age friendly.
 
This webinar will be held Monday, July 17 from noon – 3 p.m. Click here to register.
 
  • Technical Assistance Sessions
The Rush Center for Excellence in Aging will facilitate the 4M training tailored to the inpatient setting. The training will review each of the 4Ms individually, emphasizing ways in which each influences the others, as they are a set. The training will include tools for assessing and acting on each of the 4Ms, consistent with the IHI guidelines, including examples of documentation and practical integration into workflows. A library of resources for implementation will be shared.

Staff from the Rush Center for Excellence in Aging will facilitate two follow-up sessions to continue the conversation and help hospitals identify next steps to act on to align their hospitals with the 4M Framework. Hospitals will be encouraged to share how they have or plan to integrate the tools and action steps for becoming an Age Friendly Hospital.
