Coordinating Care: Mammography and Wellness Visits
Gundersen St. Joseph’s Hospital and Clinics sought to enhance patient access by streamlining mammograms and annual wellness visits. The team achieved this improvement by initiating a process where the medical imaging team reviewed patients scheduled for annual wellness visits at the beginning of each month. From this group, they identified eligible patients for mammogram screenings and proactively reached out to them. The purpose of the call was to offer the option of scheduling their mammogram on the same day as their wellness exam. This coordinated approach resulted in a remarkable 21% increase in the number of screenings, demonstrating the effectiveness of consolidating these appointments for patient convenience and optimal healthcare delivery.
