Improving Diabetes D5 Quality Metrics

In January 2023, Westfields Primary Clinic experienced a significant decline in pre-visit planning completion rates, falling to an average of 56%, well below the 100% target. Similarly, Optimal Diabetes Control (ODC) metrics fell to 42.67%, below the goal of 48.8% and their two-year high of 47.7%. Through clinic observations, interviews, and thorough data analysis, including provider comparison reviews, a clear correlation between Pre-Visit Planning (PVP) completion and ODC metrics emerged.

To address these challenges, the project team, comprising multiple frontline staff members, outlined the components of PVP and pre-charting. This facilitated the alignment and retraining of staff on processes and best practices. Leveraging data, the team garnered provider buy-in by illustrating the direct connection between PVP and optimal practice. Furthermore, the team timed their new process with planned technology updates.

Following implementation, team-based auditing led to a notable increase in PVP completion rates, subsequently resulting in improvements in ODC metrics. Despite seasonal staffing challenges impacting their ability to maintain robust PVP rates, ODC metrics consistently remain above baseline and either meet or exceed established goals.
