The Stroke and Social Determinants of Health Series is a partnership between the Wisconsin Hospital Association and the Wisconsin Coverdell Stroke program. The goal of the series to introduce health care professionals to the topic of Social Determinants of Health across a variety of areas. These areas include background on the different types of Social Determinants of Health, how they can impact stroke prevention and treatment, and information on their documentation as Z-codes within a patient’s record. This represents a broader effort by the Coverdell Stroke Program to address health equity by informing those in and out of the clinical setting of the factors that can, and do, lead to disparities in stroke incidence and stroke outcomes, which are prevalent in Wisconsin.
