Stroke and Social Determinants of Health Self-Paced Learning Series
The Stroke and Social Determinants of Health Series is a collaborative initiative between the Wisconsin Hospital Association and the Wisconsin Coverdell Stroke Program. This series aims to educate health care professionals about the impact of Social Determinants of Health (SDOH) on stroke prevention, treatment and outcomes. It covers the various types of SDOH, their influence on stroke care and how these factors are documented using Z-codes in patient records. The series is part of the Coverdell Stroke Program's broader effort to promote health equity by raising awareness of the disparities in stroke incidence and outcomes, particularly in Wisconsin.
Resources to provide support to Wisconsin hospitals to establish and enhance stroke systems of care, including strategies that meet the standards required to achieve Acute Stroke Ready Hospital (ASRH) certification, ultimately improving stroke patient outcomes across the state.
