Froedtert Holy Family Memorial Hospital remains mission-oriented and committed to providing their community with the highest level of care. With an increased focus on health care reform, the team recognized the opportunity to collaborate with hospice. When the mortality index was high, patients were receiving quality care but, unfortunately, the mortality index rate did not represent this care. Through the collaborative work with Clinical Documentation Improvement and a partnership with Sharon Richardson Community Hospice, the quality of hospital end-of-life care improved community reputation was maintained, and patient satisfaction and transitions of care improved.
