Third Webinar on Reducing Readmissions Draws Statewide Participation
On June 9, WHA held its third webinar for the readmissions quality initiative with the goal of reducing 30-day readmissions for heart failure, pneumonia, chronic obstructive pulmonary disease (COPD) and heart attack. The session drew close to 40 participants from 20 critical access and small rural hospitals’ continuing participation.
This was an opportunity for participants to revisit what they heard in the first two webinars from mentor hospitals Door County Medical Center and Marshfield Medical Center-Park Falls. The group also spent time sharing best practices while networking to gather ideas for building their 30-60-90-day action plans from mentor strategies and reviewing valuable community resources. The action plans will be reviewed in more detail in July at a follow up coaching call with report-outs of progress on their implementation plans continuing through the summer.
