WHA Partners with Rush University's Center for Health and Social Care Integration in Part 1 of Webinar Series
More than 50 people from 23 Wisconsin hospitals/organizations attended the Nov. 16 webinar, Leveraging Locally for Social Needs. Walter Rosenberg MSW, MHSM, LCSM, and Lizzi Cummings MSW, LCSW from the Center for Health and Social Care Integration (CHaSCI) at Rush University shared the impact of social care on health outcomes, provided an overview of tools to have effective social needs conversations with patients and shared how to use an Eco-map tool to create social support networks.
This was the first webinar in a two-part series sponsored by the Wisconsin Office of Rural Health focused on integration of social care to promote health, ensure equitable outcomes and reduce readmissions.
The second webinar, Social Care-Beyond Resources, is Dec. 7, 2022.
