The 2022 WHA Post-Acute Care Conference will provide an opportunity for colleagues working on patient care transitions from a variety of positions and perspectives to gather in person for the first time in several years to learn from each other and hear from a strong group of presenters.
Conference presenters will discuss leading through change, the value of Care Transitions Intervention (formerly the Coleman Model) and hospital-at-home care in Wisconsin. Attendees will also hear forward-looking insights from the administrator of the Wisconsin Division of Quality Assurance and learn from the experiences of hospitals and family care managed care organizations on how they can work together to improve care transitions.
The WHA 2022 Post-Acute Care Conference is scheduled for Aug. 5 from 8 a.m. – 3:30 p.m. in Wisconsin Dells.
