Medicine errors account for the largest number of medical errors in health care. They’re also the most-common reason for unnecessary hospital readmissions. For the first time, CMS mentions that nursing needs to be aware of the pharmacy standards. This includes medicine errors, adverse events, drug incompatibilities, self‐administered medicine and required medicine policies. This program reviews the pharmaceutical services condition of participation and other regulatory areas that require pharmacist input and participation such as antibiotic stewardships, medicine administration timing policies, medicine self-administration and standing orders.

Health system executives frequently struggle with how to summarize the success (or lack thereof) of their health systems overall performance and lack a defined metric to align organizational goals around. HSG developed Patient Share of Care in collaboration with Academic and Community Hospitals to create a single metric measuring the health system’s acquisition of patient lives and retention of those patients within their network of Inpatient, Outpatient and Ambulatory care. This webinar will cover an overview of Patient Share of Care, as well as explore alternatives for how it is calculated and utilized by health systems of varying sizes and geographies.

A University of Wisconsin–Madison School of Nursing team consulted with hospital staff and leadership, family caregivers of people living with dementia and community agencies during a two-year study. They will share those needs identified by hospital staff and family caregivers, and the team’s user-centered approach to developing dementia friendly training materials. They will also share how the resulting Dementia Friendly Hospital Toolkit can be used to develop and implement an organization-wide plan to increase staff awareness and skills to meet the needs of patients living with dementia and their family caregivers.
The Dementia Friendly Hospital Toolkit includes:
- An organizational guide that helps hospitals develop an ongoing training plan based on their goals and learning opportunities
- Eight interactive role play scenarios, with learning objectives, facilitator guides, scripts, handouts, and options for carrying out role play trainings
- Five short training videos, with learning objectives, handouts, and options for using the videos
- Additional resources that help hospitals measure progress, align their dementia friendly work with Age-Friendly Health Systems initiatives, and provide additional context on the common needs of and supports for people living with dementia and their family caregivers.
